Skipped bulk-call lines record why they were skipped
A line that never dialled now carries its reason instead of leaving you to guess.

Until now a skipped line in a campaign recorded nothing at all, so a line that never dialled looked the same whatever the cause.
Each skipped line now stores the reason it was skipped, and it shows on the line alongside the existing skipped counts and in the line listing API. Skipped stays distinct from failed, so these lines are still excluded from automatic retries.
