Recordings now recover when saving fails
A failed save is tracked and retried so completed calls are less likely to appear without a recording.

Each call now owns one recording record, saved as one transaction. If that save cannot complete, the call is marked for retry instead of silently losing its recording.
This also keeps carrier-fetched and platform-recorded calls consistent when the first delivery attempt is interrupted.
