Bulk calls recover sooner from stalled call starts
A stalled carrier request no longer holds a campaign worker for minutes.

When a call-start request stalls, the campaign now stops waiting after a bounded interval and frees the worker to continue with other calls. The limit leaves room for healthy, slower call starts while preventing one request from holding up the queue.
